REGISTRATION
ENROLLING GRADES 1-6:
Please print and complete all of the forms below for your registration packet. These forms are also available at the Sollars main office. It typically takes 3 working days for your child to begin school once the registration packet is completed and turned in to the registrar.
ENROLLING KINDERGARTENERS:
According to DS Regulation 2000.3, students must be five years old before October 31 to enter kindergarten. Please print and complete all of the forms ABOVE (except Student GR1-6 Information Form), as well as this additional form:
It typically takes 3 working days for your child to begin school once the registration packet is completed and turned in to the registrar.
In addition to the above forms, you are ready to register when you bring the following to the registrar's office:
- Copy of Sponsor's current orders
- For Navy Personnel: in addition to PCS orders, Entry Approval or Eligibility for Housing/DoDDs Form
- DEROS Verification/Certificate of Your NEW DEROS
(If you have different DEROS than listed on your initial orders) - Child’s Passport and/or Birth Certificate (Kindergarten and Grade 1 Only)
- Student's social security number
- Immunization Certificate (completed and signed by the immunization clinic at the Misawa Base Hospital)
- Local emergency contact information in the event the school is unable to reach the sponsor
